MINOR REQUIREMENTS

Required Documents:
For a minor to receive a piercing, a parent or legal guardian must be present at the time of the procedure and provide:

1. Parent's valid government issued ID/passport
2. Prove Legal Guardianship
3. Sign Required Forms: a consent form and statement affirming legal guardianship.

Acceptable forms include birth certificate (not a mother's copy), court order showing custody or guardianship, or other legal documents accepted by the Department of Health.

Age Guidelines:

• Ear & Nostril Piercings: Available for ages 12+.
• Navel Piercings: Available for ages 15+.
• Other Body Piercings: (septum, eyebrow, mouth, etc.): Available for ages 16+.

Important Notes:
A declaration acknowledging that lying about legal guardianship is a crime (Class C misdemeanor) Important: Falsifying this information is against the law and can result in up to 30 days in jail or a $50 fine.

We're required by law to keep detailed records for 2 years, and to send a copy to the Tennessee Department of Health within 30 business days. This includes:

1. Your signed consent
2. Proof of age and guardianship
3. Aftercare documentation
